Gastrografin

Gastrografin is a water-soluble, iodine-based contrast agent used for radiographic and CT imaging of the gastrointestinal tract. Because it's water-soluble rather than barium-based, it's the contrast of choice when there's any suspicion of perforation, leak, or when a patient may head to surgery — situations where barium sulfate would be inappropriate. Radiology departments, imaging centers, and surgical facilities rely on it for studies of the esophagus, stomach, and small bowel.

Clinicians choose water-soluble contrast for a few practical reasons. It clears from the body readily, it won't cause the granulomatous complications associated with extravasated barium, and its hyperosmolar properties can play a therapeutic role — most notably in the diagnosis and management of adhesive small bowel obstruction, where an oral challenge helps predict which patients will resolve without an operation. It's also standard for confirming anastomotic integrity after GI surgery and for evaluating suspected aspiration risk before a barium swallow.

Administration is typically oral or via nasogastric tube for upper GI studies, or rectally for contrast enema examinations, and dilution depends on the study and the patient. Because the agent is hyperosmolar, dilution guidance matters especially in pediatric, dehydrated, or aspiration-risk patients — always follow the prescribing information and your facility's protocol. Handling, storage, and dosing should conform to manufacturer labeling.
