What makes the Peg-Assist different from a standard insole is its removable peg construction. The insole is built from layered Plastazote, Poron, and EVA, and individual pegs can be pulled out from directly beneath a wound site. Removing pegs creates a customized void that redistributes pressure away from the ulcer, so the surrounding tissue carries the load instead. This targeted offloading is why clinicians reach for it during ulcer treatment and post-surgical recovery.

Sizing is straightforward once you know the patient's shoe size. Men's options run from the Small PTQM1 (shoe size 6–8) through the Medium and up to the Large PTQM3 (10.5–12), while women's sizing spans the Small PTQW1 (4–6), Medium PTQW2 (6.5–8), and Large PTQW3. There's also a universal square-cut style measuring 8-1/2 x 10 in that can be trimmed to fit inside a healing shoe or cast boot. Choose based on the patient's actual foot size rather than their usual insole preference, since a snug, non-sliding fit keeps the offloading pegs positioned correctly.
